Please review the below questions before proceeding with the application process!!

1. Have all your pets (past and current) received annual examinations since you've had them?

2. Have all your pets (past and current) received all core vaccinations (Rabies, Parvo/distemper) when due since you have had them (unless medically ill-advised by a licensed veterinarian)?

3. Have your pets (past and current) received titer tests in lieu of receiving the above core vaccinations?

4. Are your pets appropriately spayed/neutered according to age and breed (unless medically ill-advised by a licensed veterinarian)?

5. Have your dogs (past and current) received yearly heartworm disease testing since you've had them?

6. Do your dogs (past and current) receive heartworm disease prevention year round (unless medically ill-advised by a licensed veterinarian)?

7. Are you aware that heartworm disease prevention requires yearly testing to dispense or receive a prescription to purchase elsewhere?

Applications that don’t meet requirements or with incomplete data WILL NOT be processed. Ensure Lucky Farms Animal Rescue has permission to obtain records from your veterinarian.
